Individual Investor(s)

  • Complete application form – section 1, 2, 3A, 4, 5, 6, 7, 8, 9, 10, 11
  • Complete Identity Verification Form for individual – section 1,2,3
  • Provide original certified copy of one of the following documents. If you do not know how to certify documents, we can assist you. Please bring your original documents to our office.
    • Passport (photo page and signature page)
    • Australia Driver license (both sides)

Company investor

  • Complete application form – section 1, 2, 3B, 4, 5, 6, 7, 8, 9, 10, 11
  • Complete Identity Verification Form for company – section 1,2,3
  • Director(s) and all shareholders (who have 25% or more shares of the company) complete Identity Verification Form for individual – section 1,2,3
  • Provide original certified copy of one of the following documents for every director and shareholder (who have 25% or more shares of the company) of the company. If you do not know how to certify documents, we can assist you. Please bring your original documents to our office.
    • Passport (photo page and signature page)
    • Australia driver license (both sides)
  • A company search of the ASIC connect website. You can pay $9 to buy “Current company information” on the website of ASIC connect.

Trust investor

  • Complete application form – section 1, 2, 3B, 4, 5, 6, 7, 8, 9, 10, 11
  • Complete Identity Verification Form for unregulated Trust – section 1,2,3
  • If the trustee is a company, preparation for company’s documents is the same as the above section – company investor
    • complete Identity Verification Form for company – section 1,2,3
    • a company search of the ASIC connect website needs to be provided. see above section – company investor
    • All Directors and all shareholders (who have 25% or more shares of the company) complete Identity Verification Form for individual – section 1,2,3
    • Provide original certified copy of one of the following documents for every director and shareholder (who have 25% or more shares of the company) of the company.
      • Passport (photo page and signature page)
      • Australia driver license (both sides)
  • If the trustee is Individual(s), preparation for individual’s documents is the same as the above section – individual investor
    • all individual trustees complete Identity Verification Form for individual  – section 1,2,3
    • Provide original certified copy of one of the following documents for every individual trustee.
      • Passport (photo page and signature page)
      • Australia Driver license (both sides)
  • Original certified copy of certain pages of trust deed. (front page, schedule page, signature page)

信托投资

Super fund investor

  • Complete application form – section 1, 2, 3B, 4, 5, 6, 7, 8, 9, 10, 11
  • Complete Identity Verification Form for SMSF – section 1,2,3
  • Provide original certified copy of one of the following documents for the trustee (if individual) or the director(s) of the trustee (if company).
    • Passport (photo page and signature page)
    • Australia Driver license (both sides)
  • Original certified copy of certain pages of trust deed. (front page, schedule page, signature page)
  • A copy search of the ASIC, ATO or relevant regulator’s website (e.g. “Super Fund Lookup” at www.abn.business.gov.au)
  • If you do not know how to certify documents, we can assist you. Please bring your original documents to our office. 

养老金投资

Requirement for All investor types

  • Application form must be original application form via mailing, posting or delivering.
  • Certified document must be original certified copy via Mailing, posting or delivering.
  • FSC form can be scan or electronic copy.
  • Make sure all signatures (ID, forms) are consistent.

Additional requirement for Non-residents investors

  • If you cannot provide ABN or TFN in the application form, tax withhold is 10%. 
  • A proof of balance or bank statement needs to be provided to prove that your nominated bank detail on application form is correct.
